With form (1) also send in a copy of your I.D. showing you live in Washington State
Also thanks to the NEW Health Dept. Ruling, your Doctors Statement saying your allowed the Medical use of Marijuana now MUST
be on Tamper-Proof /Copy proof Paper.
Form 2. Notice of fraud
All Forms need to be mailed to P.O. Box 482 Colfax, WA 99111
If you can scan them in on your printer then you can attach the signed forms to Compassion 4 Patients
